I have a great idea that will make high tier players wanting to farm most dungeons if not all dungeons.
[Boss Pet System]
Implement boss pets that follows you with a uniqe boss animation to showoff ur luck/skill xD
It should be super easy to code, will add rare drop to the all dungeons.
Droprate should be around 1/100
but u could also add an achivement that gives u the pet garanted after 100 clears so people dont grind like crazy and never receieve it.

[Lakan pet] (Animation - Fake aggro Claw move
[Lachelith pet] (Animation - In out/out in move

Also the Argog weapon skin was a great way for many people to spend time grinding some older dungeon nobody does anymore,
do this to more dungeons for more content.
Armours, weapon skins, mounts to drop in dungeons.
Example:
Thaumetal refinery NM/HM: could drop something look a like to the argog weapon skin but Blue/White color
Dark Reach Citadel NM/HM: could drop something look a like to the argog weapon skin but Black/Red Color
RK9 NM/HM: Riding skill: EXtrm (recolored to a silver)
[Auras]
Uniqe character glow
(equipment slot back ornament)
(adds a glowing aura around ur character)
(this can also be added in different variants in terashop)
The glowing aura could be in the looks of wind,shadows,electricity, fire even hearts and much more.
[Titles]
Title: "No Lifer" = 100 days online time
Title: "Dungeon Expert" = 50 clears in all (Hard mode) Dungeons (Special blue/golden glow)
Title: "Boss Collector" = 5 or more (new rare boss pets aquired) (Special blue/golden glow)
Title: "Skilled" = (10 clears all dungeon) obtain skilled in all dungeons (orange title)
If you have Title: No Lifer and Title: Dungeon Expert, you can unlock title.
Title: "Master"

I dont have a whole idea of a dungeon.
but i know for sure that dungeons with Phrases are highly appriciated (bahaar and harrowhold)
You could make a boss fight with 4 phrases, all phrases dropping decent things so even if u cant clear phrase 4, u will still obtain some decent loot.
Uniqe phrases like ballistas from harrowhold was pretty cool,
how about an entire phrase (minigame) based of GVHM (pre lastboss) lazer minigame?
