I don't get why you'll even ask for an 'special' award. It's just pretty much what Greensheep is saying.

Because if you consider Dreadspire 2 earned a first clear-title and harrowhold is like 10 times harder than Dreadspire it would be veeery fair to say the least.

I agree with Chaos on that. If you were to get a first clear-title for killing bosses which already existed beforehand, you should atleast get some kind of acknowledgement for being the ones to kill an entirely new set of monsters (except for stage 1).
A title is the minimum of what we should get in my opinion.
